On the page, select “Sign up as a patient” if it’s your first time.
Fill in your name, email, date of birth, and create a password. Or streamline the process using your social media account.
Enter your implant details, including the serial number — which you'll find on your Motiva implant ID card.
If you haven't received your implant card or can't find your serial number, don’t worry. Simply get in touch with us and we’ll help you retrieve it.
When Should You Register?
As soon as possible! You must complete your registration within three months of your breast implant surgery to activate your extended warranty and financial coverage.
